The Social Worker I (Social Work Assistant) for the Homeless Prevention Program is responsible for assisting the Clinical Social Worker as part of a team to provide case management and mental health intervention activities. Services are provided to older adults who are at risk of eviction and homelessness or who are recently homeless due to mental health issues including hoarding disorder. The Social Worker I will work within a team structure along with a Clinical Social Worker and will provide care navigation, case management interventions and mental health support to clients. Field based services are provided within the client home setting with the goal of retaining housing, decreasing symptoms of mental illness and hoarding behaviors and to ensure healthy and stable living conditions.
