The Hospital of the University of Pennsylvania (HUP) is hiring a full time Transplant Coordinator RN to join the Liver Transplant team. **Position Description** : The Liver Transplant Program cares for patients and their families across the continuum of advanced organ failure and transplantation. Our program is one of the most comprehensive, high-volume centers in the country for patients with end-stage liver disease. Our Liver Transplant Team is committed to advancing our vision of collaborative patient care, education of multidisciplinary trainees, and clinical research. The Clinical Nurse 2-Transplant Coordinator is a professional nurse who practices safely in accordance with the UPHS professional practice model and demonstrates UPHS core values that reflect the organization's mission. The Clinical Nurse 2-Transplant Coordinator provides direct care and coordinates care for patients with end-stage organ failure who are referred to the Liver Transplant Program. The patients range in age from adolescents to the elderly. In this role, you will provide direction in managing patients throughout the post-transplant phases of the transplant process, perform a systematic assessment and evaluation, and develop a plan of care for the patient and their significant others. You will also serve as an integral member of the transplant team in meeting the needs of the transplant patient and ensuring continuity of care throughout the transplant process. The Clinical Nurse 2-Transplant Coordinator maintains competencies in the care of transplant patients and demonstrates the skills and knowledge necessary to provide ongoing, individualized care. The coordinator complies with all regulations and standards of regulatory and accrediting bodies. **Summary:** · The Clinical Nurse 2 - Transplant Coordinator is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model. · The Clinical Nurse 2 - Transplant Coordinator possesses the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice as specified in the Clinical Nurse 2 position description addendum. · The Clinical Nurse 2 - Transplant Coordinator exhibits UPHS organizational behavioral competencies which reflect its mission and are embedded in the professional practice competency domains. · The Clinical Nurse 2 - Transplant Coordinator independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. The age of patients can range from adolescents to the elderly. · The Clinical Nurse 2 - Transplant Coordinator demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork (Definitions listed below). · The Clinical Nurse 2 - Transplant Coordinator Colleague works closely with the multidisciplinary transplant team to facilitate the coordination of care across the continuum. The nurse focuses on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age appropriate considerations of assigned patients. · The Clinical Nurse 2 - Transplant Coordinator seeks feedback in assuming leadership roles and provides feedback for improved clinical practice. The nurse demonstrates how nursing practice impacts the organizational vision, mission, and goals and the care delivery system. The nurse complies with all regulations and standards of regulatory and accrediting bodies. Some rotational on-call duties. Hours may be adjusted to meet the department's operational needs.
